REVIEW OF THE SALVAGE PROCEDURE IN THE DIGITAL REPLANTATION

Abstract
Komotsu and Tamai opened the era of digital replantation by microvalscular anastomosis in 1965 with the successful replantation of a competely severed thumb.

During the following 20 years, the success rate of replantated digit has been increased, however, the management of amputation distal to the DIP joint and special circumstances such as impossible venous anastomosis are still difficult and their survival rates are guarded.

Generally, the replantation of the amputation distal to the DIP joint is difficult because of the small caliber of vessels, especially veins. In such case, we try replantation by only arterial anastomosis and we call this technique as salvage procedure.

We present 26 years cases of salvage procedure which has been performed at the Department of Plastic Surgery, National Medical (1980~1985) which its review and analysis .

The results were as follows:

1. Survival rate after salvage procedure was 77% to be remarkable good result.

2. Common cases were guillotine type and complete amputation to be involved index and thumb in ten £¦ twenty decades.

3. Patient with bleeding was frequently transfused less than 5 points of W/B in zone ¥± , but 5 to 10 pints of W/ B in zone ¥².
